Billing & Insurance
Paperwork:
In some cases it is necessary for me to be able to converse with other health
professionals that are involved in your care, view lab results, etc.  In these cases, I
would ask that you complete a release form so that I may request this information
or contact another of your providers (MD, ND/NMD, therapists, etc.)  In the same
respect, all of the work that we do together is confidential unless a release form is
completed by you to allow me to share information with another provider.

I also work with adolescents and minors.  If you would like for me to work with
anyone under the age of  18, I will need a completed parental consent form prior
to our initial visit.


Billing:
I do ask that all fees are paid at the time of service for in-house clients & in
advance for email and telephone clients.  I currently accept cash, personal checks,
& credit/debit cards (Visa, MasterCard, & Discover).

Fees vary for each client depending on need and diagnosis.  Initial consults are
usually 1.5hrs and follow up consults about 45min.  I do charge for no-shows or
cancellations with less than 24hrs notice.  Please contact me for fees and pricing.

Insurance:
I currently do not file with Insurance companies, although I am a Registered
Dietitian which can be approved by many health insurance companies.  I will
present you with a letter to submit to your insurance company if requested.
